Terms and conditions for Super Match searches

By ticking this box you consent to FSS Trustee Corporation ABN 11 118 202 672, AFSL 293340, as the trustee of the First State Superannuation Scheme ABN 53 226 460 365 (First State Super) using your tax file number (TFN) to search Australian Taxation Office (ATO) records to locate superannuation amounts held on your behalf by other superannuation funds or by the ATO now and in the future.

If search results include superannuation amounts held on your behalf by the ATO, you also consent to First State Super transferring these amounts into your First State Super account and using and disclosing your TFN for that purpose.

Once you provide your consent it will be held for five years, this will allow First State Super to complete periodical searches on your behalf of which you will be notified. If at any time you wish to revoke your consent please contact First State Super on 1300 650 873. Tax file number (TFN)

Under the Superannuation Industry (Supervision) Act 1993, your superannuation fund is authorised to collect your TFN, which will only be used for lawful purposes. These purposes may change in the future as a result of legislative change. The trustee of your superannuation fund may disclose your TFN to the Commissioner of Taxation or to another superannuation provider when your benefits are being transferred, unless you request the trustee of your superannuation fund in writing that your TFN not be disclosed to any other trustee.

It is not compulsory to provide your TFN but if you don’t, you may pay additional tax and your contributions may not be accepted.

If you don’t provide your TFN or consent to use your TFN, we will not be able to use the ATO SuperMatch facility to search on your behalf.


The personal information you provide on this form is collected by and held for First State Super by the fund administrators, Pillar Administration and Chandler, in accordance with the Australian Privacy Principles of the Privacy Act 1988 (Cth), for the purpose of administering your account and providing you with services associated with your fund membership.
